This text provides information about the processing of personal data in the contact and appointment request forms on our website. Separate information is provided for health records kept during examination and treatment. Submitting a form does not constitute a confirmed appointment or a medical assessment.

What information is processed in the forms?
In the contact form, full name, phone number, e-mail, subject and message are recorded; in consultation requests on treatment pages, full name, phone number, the procedure of interest and, if you write one, the message are recorded. The application date, procedure status and the version of the displayed information notice are also associated with the record. Hashed values derived from the IP address and browser information may be used to prevent misuse.
Do not send diagnoses, medical history, test results, medication information, identity documents or patient photographs through the forms. These forms are not designed to collect health documents or to obtain emergency assistance.
Purpose and legal grounds
Your contact information and request are used to answer your question, to contact you and to schedule a consultation at your request. To the extent that the evaluation of your service request is directly related to the establishment or performance of a contract, processing is carried out under KVKK Article 5/2-c; technical logs concerning system security and the prevention of misuse are processed under Article 5/2-f, provided that they do not harm your fundamental rights and freedoms. The preservation of records necessary in a legal dispute may be considered under Article 5/2-e.
Your indication that you have read the information notice does not replace consent for marketing or explicit consent for processing health data. Submitting a form application does not register you on a marketing list.
Collection method and recipients
Information is received electronically from the content you enter and submit in the form fields and is kept in the site's application record system. Authorized clinic personnel may access applications within the scope of their duties. Site hosting and e-mail service providers may be involved in the processing to the extent required by their technical services. Information may be disclosed to competent public authorities only when legally required.
The website's application notification is intended to direct the team to the record in the management panel. If you use the WhatsApp link, WhatsApp's own data processing terms will also apply; using this link is not mandatory to submit the form.
Retention
The retention of applications is based on the time required to fulfill the request, applicable legal obligations and the periods necessary to protect a right. Deletion, destruction or anonymization processes should be applied to data whose processing purpose has ceased. This notice does not contain an automatic deletion commitment on a specific date; you may contact us regarding the status of your record.
Your rights and application
Under KVKK Article 11, you have the rights to learn whether your data is being processed; if processed, to request information; to learn the purpose and whether it is used in accordance with that purpose; to know the recipients to whom it has been transferred; to request correction of incomplete or inaccurate information and, where conditions apply, its deletion or destruction. You may request that these actions be notified to recipients, object to an adverse result arising solely from automatic analysis, and seek compensation for damage due to unlawful processing.
